Note: This registration is a binding, legal document of your generous gift. You affirm the information provided is accurate. You agree upon death to donate all eligible organs, eyes and tissue. This is referred to as First Person Authorization.
Adding your name is simple:
- Check for the symbol on your license or state ID.
- If you don’t have one, complete the form here to join the Donor Registry.
- After you join you’ll receive a link to request the donor symbol from the Secretary of State for your state ID.
